The division of Quality and Population Health measures, analyzes, and reports health outcomes of Kentucky Medicaid members. The branches comprising this division are Quality, Population Health, Research and Analytics, and Behavioral Health Innovations. These branches work together to achieve optimum health outcomes for all Kentucky Medicaid members.
Division News
In August 2024, the Division Quality and Population Health (DQPH), within the Department for Medicaid Services (DMS), presented at the national Medicaid Enterprise Solutions Conference (MESC) – Obtaining and Connecting Social Determinants of Health (SDoH) Data. DMS, with other Cabinet of Health and Family Services agencies, developed a system to connect Medicaid members with additional needed resources to help improve health outcomes. Continuous Glucose Monitor Access Accelerator Program
In 2023, Kentucky was selected to participate in the Center for Health Care Strategies Continuous Glucose Monitor Access Accelerator program supported by The Leona M. and Harry B. Helmsley Charitable Trust.
The program focused on ways to increase access to continuous glucose monitors and improve diabetes care for Medicaid members. A continuous glucose monitor is a wearable device that checks blood glucose levels throughout the day. It gives people information about their blood glucose levels in real time. This information can help people with diabetes and their health care providers make better decisions about diabetes care. Better management of diabetes helps improve long-term health outcomes related to diabetes.
Project Accomplishments
Expanded continuous glucose monitor coverage to include members with gestational diabetes, even if they do not use insulin.
Expanded continuous glucose monitor coverage to include members with Type 2 diabetes who do not use insulin but have a history of serious low blood sugar.
Created diabetes resources for members, caregivers, and providers.
Worked with kynect resources to create a Diabetes category with information about local programs and support.
Shared Kentucky's work through a Center for Health Care Strategies webinar. The webinar covers efforts to expand access to continuous glucose monitors and automated insulin delivery devices in Medicaid.
Next Phase
In January 2026, the Center for Health Care Strategies selected Kentucky to take part in the next phase of the project. This phase will focus on continuous glucose monitors and automated insulin delivery devices. The project runs from fall 2026 through 2027.
