Supply Kit Ordering
Ordering Division of Laboratory Services supplies has officially transitioned to the Kentucky Online Gateway (KOG). If you do not have access to the KY DLS Portal on your KOG homepage, please email leighann.bates@ky.gov. Please be aware that email and fax are no longer the preferred methods for submitting orders.
The Requisition Form for Ordering Lab Kits is only to be used when KOG is unavailable. Email completed forms to dphlabkits@ky.gov

Laboratory Submission Forms
The Division of Laboratory Services strongly prefers electronic orders through our electronic ordering and reporting systems (Outreach and Telepathy). Paper forms should only be used when access to these systems is unavailable. These forms collect important details like patient information, test requests, specimen type, and provider contact information. Completing forms accurately and in full helps prevent delays and ensure timely, reliable results.
