The Kentucky Division of Laboratory Services (KY DLS) Outreach Web Portal provides approved partners with streamlined access to laboratory test ordering and related services for clinical and environmental testing. This secure, web-based system is designed to simplify submissions, improve communication, and support efficient public health laboratory operations. Below are instructions for requesting access to the portal.

To register, complete and return to the lab the following forms and return to the address on the user request form:
When your registration forms are received, processed, and approved, you will become a certified user and be provided with your Outreach system user credentials and instructions.

contact our lab during regular business hours (Monday-Friday 8 a.m. to 4:30
p.m.) at (502) 564-4446 or via email with questions about registration,
specimen submission, order submission, and result retrieval, or Outreach
IT-related issues. Calls and emails received outside regular business hours
will be responded to on the next business day
