All requests for records must be signed by the requestor.
The following information may be released by the Division of Health Care:

• Type A or B Citations

• Correspondence
The Division of Health Care has three business days from the date of receipt to respond to an open records request. If the documents requested are not available for release within those three days, a response will be provided informing the requestor of the approximate date by which to expect the requested documents.
NOTE: If the records sought are under the custody and control of another agency, you would need to submit your request to that agency. For example, the following information is under the custody and control of the Centers for Medicare and Medicaid Services:
• Surveyor notes or worksheets regarding Medicare/Medicaid-certified health care facilities
• Complaint narratives regarding Medicare/Medicaid-certified health care facilities
If you wish to secure copies of surveyor notes/worksheets or complaint investigations for Medicare/Medicaid-certified health care facilities, you will need to submit a Freedom of Information Act request to:
