Nursing Home Survey Inspection Findings

The Office of Inspector General is Kentucky's regulatory agency for licensing all long-term care facilities in the commonwealth. Below you will find information for each certified long-term care facility:

• Ownership Information - This information may be found by viewing the provider's application. You may click the provider name for a copy of the application. Note ownership information available for persons with 25 percent or greater ownership. Once the Centers for Medicare and Medicaid Services (CMS) defines requirements, the level of information may change.  
• Survey Findings - The link under the Statement of Deficiency (SOD)/Plan of Correction (POC) reflects a date indicating the date the survey or inspection concluded. Click the link to review survey findings for the date specified.
