**NOTE:  If you have not applied for Local Health Department positions before, please read the information below before completing the Application Form. Once form is completed, please return to the local health department where employment is being sought for proper consideration.**

Application Form & Information

The various health departments throughout the state will announce vacancies from time to time to recruit for qualified individuals to work in the field of public health. In order to be considered for a position with a local health department, an application for employment (CH-36) must be completed and submitted within the time frame(s) required by the local health department in its announcement. The application itself has instructions on completion and the submission of the application to the local health department of interest.

Once the application has been submitted, staff of the Local Personnel Branch will review the information and material to determine if an applicant meets the minimum requirements of education and experience. An applicant will be notified by mail of the results of the review. If an applicant meets the minimum requirements for the specific position, the applicant’s name will be placed on a register, which is a listing of applicants certified as meeting the minimum requirements of education and experience. A local health department uses the register to select an applicant for the vacant position. An applicant will remain on the register for a period of one year from the date of establishment of the register, unless the health department depletes the register and/or advertises again for the position. In such cases an applicant will need to re-apply or contact the local health department for consideration.

The majority of announcements for vacancies in a local health department, are considered scheduled, meaning that applications will be received during a specified time period, thus closing the recruitment effort There are certain positions in health departments, that because of recruitment difficulties, recruitment efforts will remain open. During this time, applications will be received at any time until such time as the position(s) is filled.

An application may be rejected by the Department for Public Health for other reasons identified in administrative regulation 902 KAR 8:070 Section 1 (6). An applicant who has taken an examination may appeal his rating in any part of an examination to assure rating procedures have been applied fairly and equitably. The appeal shall be in writing and mailed to the department no later than thirty (30) days after the date on which notification of the results of the examination was mailed to the applicant.

An eligible whose name has been removed from a register for any of the reasons specified in administrative regulation 902 KAR 8:070, Section 1(6) may appeal the action. The appeal form shall be mailed to the department within thirty (30) days after the date on which the notification of removal was mailed to the eligible.
