KY-EPHRS User Request Form
Use this form to request access to the Disease Surveillance Application. This form must be completed, signed by your supervisor/manager and returned by Postal Mail to the address listed above. No faxes will be accepted.
HIPAA Agreement Form
This form must be completed, signed and returned along with the User Request Form in order to be given access to the KY-EPHRS System.
KY-EPHRS Change Request Form
Use this form for requesting changes to the Disease Surveillance Application. Requested changes can be for errors, misspelling, workflow, and enhancements.
